ABSTRACT

ABSTRACT Introduction: Relapse of acute myeloid leukemia (AML) after allogeneic stem cell transplantation (allo-SCT) leads to dismal outcomes. This study aimed to identify high-risk patients and explore the effects of cytomegalovirus (CMV) reactivation in a high CMV-seropositive population. Methods: The study involved a single-center retrospective cohort in Thailand, analyzing clinical risk factors and CMV-mediated immune responses, correlated with transplant outcomes in AML patients. Results: Eighty-five patients with AML in complete remission (CR) undergoing HLA-matched myeloablative allo-SCT between 2011 and February 2021 were enrolled. The relapse rate was 27.1% with the median time of 7 months after transplantation. The 3-year relapse-free-survival (RFS) and overall-survival (OS) were 72.2% and 80.8%, respectively. The disease status (>CR1) and absence of chronic graft-versus-host disease (cGVHD) were independently significant adverse prognostic factors of RFS and OS. Ninety-two percent of recipient-donor pairs were both CMV seropositive. The CMV reactivation occurred in 54.1% of the patients. The clinically significant CMV infection rate was 49.4%. No CMV syndrome/disease or CMV-related mortality occurred. One-year cumulative incidence of relapse among CMV-reactivation and non-reactivation groups were 14.3% and 25.6%, respectively, without a statistically significant difference. Transplantation-related mortality was 11.1%. Conclusions: The transplantation beyond CR1 and absence of cGVHD are powerful prognostic factors associated with inferior RFS and OS. In a high CMV prevalence country, there appears to be no impact of CMV reactivation on relapse in AML patients undergoing an allo-SCT.

【Objective】 To investigate the correlation between early immune reconstitution and clinical outcomes in patients with acute lymphoblastic leukemia (ALL) underwent allogeneic hematopoietic stem cell transplantation (allo-HSCT). 【Methods】 The basic information and treatment data of 99 patients with ALL undering allo-HSCT from December 2018 to February 2022 were collected. The proportions of CD3+ T, CD3+CD4+ T, CD3+CD8+ T and CD3-CD16+CD56+ NK cells were detected before and 30, 60 and 90 days after transplantation using flow cytometry. The correlation between early cellular immune reconstitution and neutrophil engraftment, platelet engraftment, infection, and acute and chronic graft-versus-host disease (GVHD) was analyzed. 【Results】 Among 99 ALL patients, the median time of neutrophil engraftment was day +11 (range, 8-28), and the median time of platelet engraftment was day +14 (range, 10-120). The cumulative incidence of blood stream infection (BSI) was 11.10% and the cumulative incidence of CMV within 100 days of transplantation was 40.40%. The cumulative incidence of EBV within 100 days was 7.10%. The cumulative incidence of acute graft-versus-host disease (aGVHD) was 22.30%. The cumulative incidence of chronic graft-versus-host disease (cGVHD) within 1 year of transplantation was 16.20%. 1 -year cumulative relapse rate was 13.84%. The 1 -year cumulative disease-free survival (DFS) for all patients was 80.60% and the 1-year overall survival (OS) was 90.30%. The CD4+/CD8+ ratio was positively associated with the development of aGVHD at 30 days post-transplant (OR 1.21, 95CI 1.01-1.45, P<0.05). The proportion of CD16+ CD56+ NK cell were higher in the group without BSI than that in the BSI group before and 30 days after transplantation (P < 0.05). The proportion of CD4+ T-cell were lower in the CMV infection group than that in the group without CMV infection at 60 and 90 days post-transplant(P<0.05). The higher level of CD4+ T-cells at 60 days post-transplant was a protective factor for CMV infection within 100 days (HR 0.91, 95CI 0.84-0.99, P<0.05). 【Conclusion】 Early immune reconstitution after allo-HSCT in patients with ALL is associated with aGVHD, CMV and BSI.

Abstract Introduction: Members of the Herpesviridae family have been described in patients with systemic lupus erythematous (SLE), but the clinical impact on renal function is not well known. Methods: HSV1, HSV2, VZV, EBV, CMV, HHV-6, HHV-7, and HHV-8 were evaluated by molecular biology on admission in blood samples from 40 consecutive SLE patients hospitalized for lupus activity. Results: Patients were 90.0% female, 77.5% non-white, with average age of 32.7 ± 13.6 years. We found positivity for EBV (65.0%), CMV (30.0%), HSV-1 (30.0%), HHV-6 (12.5%), and HHV-7 (7.5%). For all viruses, age, SLEDAI, hematological tests, ferritin, LDH, C-reactive protein, and erythrocyte sedimentation rate (ESR) were not significant. However, EBV positivity was a significant factor for higher serum creatinine (3.0 ± 2.8 vs. 0.9 ± 0.8; P = 0.001) and urea (86 ± 51 vs. 50 ± 46; P = 0.03). Moreover, positive cases for EBV only or with combined co-infections (66.7%-CMV; 58.3%-HSV-1) or negative for EBV only were evaluated by Kruskal-Wallis test again showed statistical significance for serum creatinine and urea (both P ≤ 0.01), with posttest also showing statistical differences for renal dysfunction and EBV presence (alone or in combined co-infections). The presence of EBV viral load was also significant for nephrotic-range proteinuria, renal flare, and the need for hemodialysis. Conclusion: Members of the Herpeviridae family (mainly EBV, HSV-1 and CMV) are common on hospital admission of SLE patients, reaching 65% for EBV, which seems to be associated with renal dysfunction and could reflect a previous association or overlapping disease, which is not well understood.


Resumo Introdução: Membros da família Herpesviridae tem sido descritos em pacientes com lúpus eritematoso sistêmico (LES), mas o impacto clínico na função renal não é bem conhecido. Métodos: Avaliou-se HSV1, HSV2, VZV, EBV, CMV, HHV-6, HHV-7, HHV-8 por biologia molecular na admissão em amostras sanguíneas de 40 pacientes com LES consecutivos hospitalizados por atividade lúpica. Resultados: Pacientes 90,0% mulheres, 77,5% não brancos, idade média 32,7 ± 13,6 anos. Encontramos positividade para EBV (65,0%), CMV (30,0%), HSV-1 (30,0%), HHV-6 (12,5%), HHV-7 (7,5%). Para todos os vírus, idade, SLEDAI, exames hematológicos, ferritina, LDH, proteína C reativa, velocidade de hemossedimentação não foram significativos. Entretanto, positividade para EBV foi estatisticamente significativo para creatinina (3,0 ± 2,8 vs. 0,9 ± 0,8; P = 0,001) e ureia (86 ± 51 vs. 50 ± 46; P = 0,03) séricas mais elevadas. Ademais, casos positivos para EBV isolado ou com coinfecções combinadas (66,7%-CMV; 58,3%-HSV-1) ou negativos apenas para EBV foram avaliados pelo teste Kruskal-Wallis e novamente mostraram significância estatística para creatinina e ureia séricas (ambas P ≤ 0,01), com pós-teste mostrando também diferenças estatísticas para disfunção renal e presença de EBV (sozinho ou em coinfecções combinadas). A presença de carga viral do EBV também foi significativa para proteinúria de faixa nefrótica, inflamação aguda, necessidade de hemodiálise. Conclusão: Membros da família Herpeviridae (principalmente EBV, HSV-1, CMV) são comuns na admissão de pacientes com LES, chegando a 65% para EBV, que parece associar-se à disfunção renal podendo refletir associação prévia ou doença sobreposta, o que não é bem compreendido.

ABSTRACT

Purpose: To study clinical efficacy of valganciclovir in cytomegalovirus retinitis (CMVR) in human immunodeficiency virus (HIV)?positive?positive patients in a tertiary care clinic in a developing nation. Methods: In a retrospective study, systemic and ocular records of HIV patients suffering from CMVR and treated with valganciclovir, were analyzed. Primary outcome measures were involvement of the other eye, incidence of retinal detachment, systemic involvement, and mortality encountered. Secondary outcome measures included change in BCVA. Results: Out of nine patients who were included, two patients developed CMVR in the other eye and only one patient (11.11%) developed retinal detachment during the course of the study. No patient developed any systemic manifestations or had mortality during the course of the study. The change in BCVA was not statistically significant. Conclusion: Use of oral valganciclovir showed good outcome and was found to be a better alternative compared to the use of intravitreal ganciclovir in the literature. Introduction of valganciclovir at an affordable price in developing nations can decrease disease burden

Purpose: To evaluate the correlation of quantitative real?time polymerase chain reaction (qRT?PCR) to the clinical characteristics of patients with viral retinitis.Methods: Retrospective case series. Results: Aqueous or vitreous samples of 20 out of 35 eyes showed qRT?PCR positivity for virus etiology (57.14%). Cytomegalovirus (CMV) was most commonly identified in nine eyes (45%). The mean DNA copy number was 2,68,339.65 copies/mL (range: 90–3205397). DNA copy number significantly correlated with the extent of clinical involvement (P = 0.013); however, there was no correlation between DNA copy number and presenting visual acuity (P = 0.31), macular involvement (P = 0.675), optic nerve involvement (P = 0.14), and development of retinal detachment (P = 0.73). There was a significant correlation between the number of DNA copies and the timing of sampling (P = 0.0005). Samples taken earlier in the course of the disease had higher viral copies than later ones. Conclusion: qRT?PCR is useful in confirming a viral etiology in over 50% of cases of suspected viral retinitis. It correlates well with the extent of clinical involvement and timing of sampling

ABSTRACT

A 62-year-old female diabetic recovered from COVID-19 pneumonia after receiving a prolonged course of steroids. She presented with a clinical picture of left-eye panuveitis with white cotton ball chorioretinal lesions and RAPD suggesting an optic neuropathy (VA HM). Diagnostic vitrectomy was performed to take samples for infective screen and to give intravitreal voriconazole empirically. Smear, culture, and PCR for viral DNA confirmed mixed infection of endogenous Candida endophthalmitis and incidental CMV infection. With further treatment, her corrected vision improved to 6/18 with regressing fungal lesions in serial fundus photographs. Prompt diagnosis and intervention preserved her vision and prevented potential life-threatening complications

Objective To evaluate the diagnostic value of quantitative detection of cytomegalovirus (CMV) DNA from different sources [plasma, sputum and bronchoalveolar lavage fluid(BALF)] for CMV pneumonia after allogeneic hematopoietic stem cell transplantation. Methods Clinical data of 405 recipients undergoing allogeneic hematopoietic stem cell transplantation were retrospectively analyzed. Among them, 19 recipients diagnosed with CMV pneumonia were assigned into the CMV pneumonia group, and 229 recipients with CMV viremia alone, 11 recipients without CMV pneumonia who received fiberoptic bronchoscopy and 16 recipients diagnosed with bacterial or fungal pneumonia based on pathogenic evidence receiving sputum culture were assigned into the control A, B and C groups, respectively. The incidence of CMV pneumonia was summarized. The CMV DNA load of specimens from different sources (plasma, sputum and BALF) of recipients with CMV pneumonia was analyzed. The clinical prognosis of recipients with CMV pneumonia was evaluated. Results Among 405 recipients undergoing allogeneic hematopoietic stem cell transplantation, 19 cases developed CMV pneumonia, and the overall incidence of CMV pneumonia was 4.7%(19/405). The CMV DNA load in the plasma, sputum and BALF of recipients with CMV pneumonia was higher than those in the control A, B and C groups (all P < 0.05). In the 19 recipients, 12 cases were cured after antiviral treatment and 7 died from treatment failure(3 cases abandoned treatment). The fatality was 37%(7/19). Conclusions Quantitative detection of CMV DNA in the plasma, sputum and BALF may increase the diagnostic rate of CMV pneumonia, thereby improving clinical prognosis of recipients undergoing allogeneic hematopoietic stem cell transplantation.

ABSTRACT

In order to prepare human-mouse chimeric cytomegalovirus-immunoglobulin M (CMV-IgM) in vitro and study the effects of different signal peptides on the secretion of CMV-IgM, genes were amplified from hybridoma cell line using RLM-RACE to construct the expression vector of chimeric CMV-IgM. Then, the signal peptide of SigF itself was replaced by five different secreted signal peptides (SigA-SigE) by PCR method, and the CHO cell was chosen as host cell for in vitro expression. SDS-PAGE, SEC-HPLC and ELISA experiments were carried out to evaluate the protein expression level and immunoreactivity of the purified CMV-IgM. A 910 kDa recombinant protein was successfully prepared and signal peptides (SigA-SigE) had an increased expressed CMV-IgM, which were 6.72, 5.19, 1.44, 1.85 and 1.98 times higher than that of the CMV 6# cell signal peptide SigF. In summary, this work provides a theoretical basis for the development of human-mouse chimeric CMV-IgM, and a novel route to increase the expression level of CMV-IgM.

ABSTRACT

Maintenance of sinus rhythm is superior to incidence of atrial fibrillation (AF) in patients with rheumatic heartdisease undergoing Closed Mitral Valvotomy (CMV). The present study was done to evaluate the effect ofprophylactic use of Magnesium Sulphate (MgSo4), intravenously (i.v) soon after opening the stenosed MitralValve using Tubbs dilators by Surgeon, in patients undergoing CMV. One hundred and twenty patients withMitral Stenosis (MS) (mitral valve area < 1cm2), normal mitral valvular apparatus, no mitral regurgitation,mitral score not more than 7/16 planned for CMV. These patients were divided in two groups of 60 each.Group (I) (n=60) received. MgSo4, 30 mg/kg diluted to 20 ml with normal saline soon after mitral valvotomyand Group - II (n=60) received 20 ml of normal saline. The standardized protocol for CMV was maintainedfor all the patients. Before surgery AF was observed in 56.67% of patients in group I and 48.33% of patientsin group II (p=0.46). Postoperatively in ICU, 30% of patients in group I and 70% of patients in group II hadAF(p<0.0001). A single prophylactic intra operative dose of i.v MgSO4 decreased post valvotomy arrhythmiain comparision to placebo group . Thus, a single dose of intraoperative MgSO4 can be used to decreasepostoperative arrhythmias in patients of M.S undergoing CMV

Cytomegalovirus(CMV) infection may be acquired congenitally, perinatally or postnatally in babies. Congenital and perinatal CMV infection can be diagnosed by viral culture or detection and quantitation of CMV DNAby Real Time Quantitative PCR (RT-qPCR) in blood ,urine and body fluids. The objective of this study was to diagnose and determine CMV load in infants presenting with clinical features suggestive of cytomegalovirus infection by RT-qPCR of urine. This descriptive study was done on babies admitted to the Departments of Neonatology and Paediatrics Govt Medical College, Kozhikode from January 2015 to December 2017. Urine samples from 142 babies were received and processed in the Microbiology Department. DNA isolation and amplification was performed using commercial DNA extraction kit and PCR kit for detection and quantification of CMV. Serum samples of the babies with CMV viruria were tested for CMV IgM antibodies. Of 142 babies suggestive of CMVinfection CMV-DNAwas detected and quantitated in urine of 25 (17.60%) (mean age 3.36 months). . CMVIgM was positive in 15/25(60%) babies with viruria .Twenty two had congenital CMV infection (cCMV) and 3 had perinatal infection.The most common clinical presentation was jaundice 13( 52%). Of 8 babies started on Ganciclovir 7 responded to treatment. RT-qPCR helps in diagnosing and quantitating CMVload which helps in deciding on therapy and assessing response to treatment,and can predict risk for long term sequelae.

Background & objectives: Ocular manifestations in HIV/AIDS patients range from adnexal disorders to the posterior segment disease. This study was aimed to evaluate the ocular manifestations, including vision-threatening manifestations in HIV-positive patients attending an antiretroviral therapy centre (ART) of a tertiary care hospital in north India and its association with the CD4+ cell count. Methods: This cross-sectional study was conducted in the department of Ophthalmology in collaboration with the ART centre. An equal number of patients were selected from each year i.e., 30 patients each from those registered in the year 2010 till 2015. These patients were selected randomly from the register using systematic randomization. Hence, a total of 150 patients were examined for ocular manifestations. All the patients included in this study were on highly active ART. Results: Of the 150 patients examined, 53 per cent were females and 47 per cent were males. Heterosexual transmission was the most common mode of transmission in 126 (84%) patients. Maximum number of patients was in the age group of 31-40 yr. Ocular manifestations were present in 53 [35.3%; 95% confidence interval (CI): 28-43%] patients. Twelve (8%; 95% CI: 4-12%) patients had lid and adnexal manifestations. Anterior-segment manifestations were present in 20 (13.3%; 95% CI: 8-19%) patients. Posterior-segment manifestations were present in 21 (14%; 95% CI: 8-20%) and vision-threatening posterior-segment ocular lesions were present in 14 per cent of the patients. Univariate logistic regression showed a significant (P<0.001) inverse association of CD4+T-cell count with the vision-threatening posterior-segment ocular lesions. Interpretation & conclusions: Routine ocular examination may be done in all the HIV/AIDS patients to detect and treat vision-threatening ocular lesions at the earliest.

Las complicaciones secundarias a las infecciones por citomegalovirus (CMV) y virus de Epstein Barr (EBV) constituyen las principales causas de morbilidad y mortalidad por infecciones en los pacientes que reciben injertos de células progenitoras hematopoyéticas (CPH). La detección de anticuerpos específicos contra cada uno de estos virus en el periodo pretrasplante permite prevenir la primoinfección durante la inmunosupresión terapéutica.La investigación tuvo como objetivo determinar la seroprevalencia de anticuerpos anti-CMV y anti-EBV e identificar aquellos pacientes con riesgo de adquirir una infección primaria postrasplante.Se realizó la detección de anticuerpos IgM e IgG anti-CMV y anti-EBV por ELISA, a 110 pacientescon indicación de pruebas de histocompatibilidad para trasplante de CPH, entre enero del 2013 y enero del 2016, en el Departamento de Histocompatibilidad del Instituto de Hematología e Inmunología de La Habana.La seroprevalencia de anticuerpos anti-CMV en la población estudiada fue del 84,5 por ciento y anti-EBV, del 90,9 por ciento. Los menores de 9 años presentaron un porcentaje de positividad del 70,6 y 64,7 por ciento para IgG anti-CMV y EBV respectivamente, encontrándose un incremento de la seroprevalencia con la edad.La seroprevalenciade anticuerpos anti-CMV y anti-EBV en los pacientes en espera de trasplante de CPH en Cuba es semejante a otras poblaciones; lo que sugiere la necesidad evitar el contagio por transmisión a los casos seronegativos(AU)


Secondary complications due to infections caused by Cytomegalovirus (CMV) and Epstein Barr virus (EBV) are the major infectious causes of morbidity and mortality in recipients of hematopoietic stem cell (HSC) grafts. Detection of specific antibodies against each of those viruses in the pre-transplant period allows the prevention of the primary infection during the immune suppressive therapy.The aim of the investigation was to determine seroprevalence of antibodies against CMV and EBV and also to identify the patients in risk of a primary infection in the post-transplant period.The investigation was conducted between January 2013 and January 2016 by the Histocompatibility Department of Instiute of Hematology and Immunology. Antibodies IgM and IgG against CMV and EBV were tested by ELISA in 110 patients with the indication for histocompatibility testing for a HSC graft.Seroprevalence of antibodies against CMV in this population was 84.5 percent. and seroprevalence of antibodies against EBV was 90.9 percent. The 70.6 percent.of children less than 9 years old had positive IgG CMV antibodies and the 64.7 percent. of them had positive IgG EBV antibodies. An increase of seroprevalence with age was found. The seroprevalence of antibodies against CMV and EBV in patients waiting for a HSC transplant in Cuba is similar to the populations in other countries. This result suggests the need of avoiding the transmissiontoseronegative recipients(AU)

Cytomegalovirus (CMV) is a well-established cause of morbidity and mortality in pediatric recipients of allogeneic hematopoietic stem cell transplantation (allo-HSCT). CD8⁺ T-cells are important for controlling CMV infection. We conducted a prospective pilot study to investigate the clinical utility of measuring the CMV-specific T-cell immune response using the QuantiFERON-CMV assay (QF-CMV) in pediatric allo-HSCT recipients. Overall, 16 of 25 (64%) patients developed CMV infection. QF-CMV was evaluated in these 16 patients during the early and late phases of the first CMV infection post allo-HSCT. Whereas the initial QF-CMV results during the early phase of CMV infection did not correlate with the course of the corresponding infection, the QF-CMV results post resolution of the first CMV infection correlated with the recurrence of CMV infection until 12 months post allo-HSCT; no recurrent infections occurred in the four QF-CMV-positive patients, while recurrent infections manifested in five of eight QF-CMV-negative (62.5%) and all three QF-CMV-indeterminate patients (P=0.019). In spite of the small number of patients examined, this study supports the potential application of monitoring CMV-specific T-cell immunity using the QF-CMV assay to predict the recurrence of CMV infection in pediatric allo-HSCT recipients.

Introducción: las infecciones virales postrasplante de órganos sólidos constituyen las principales causas de morbilidad y mortalidad de los pacientes trasplantados. En Cuba se introdujo recientemente la detección de anticuerpos clase IgM e IgG, antivirus de Epstein Barr (EBV) y anticitomegalovirus (CMV) mediante técnicas de ELISA con analizador automático como parte del aseguramiento pretrasplante renal. Objetivo: determinar la prevalencia de las infecciones en los pacientes en espera de trasplante renal y si existe asociación entre la presencia de anticuerpos anti-EBV y anti-CMV con posibles eventos sensibilizantes y la presencia de anticuerpos anti-HLA. Métodos: se estudiaron 1 179 muestras de pacientes en espera de trasplante renal, entre agosto de 2013 y diciembre de 2014. Se realizaron 4 técnicas de inmunoensayos enzimáticos (ELISA) de tipo heterogéneo, no competitivo, cuantitativo e indirecto usando los estuches comerciales: Cytomegalovirus IgG ELISA, Cytomegalovirus IgM ELISA, Epstein-Barr virus VCA IgG y Epstein-Barr virus VCA IgM. El estado de aloinmunizacion anti-HLA clase I y II se definió de acuerdo a los estudios realizados por ELISA con los estuches comerciales: LIFECODES QuikScreen y LIFECODES B-Screen. Se empleó el estadígrafo Chi cuadrado de independencia para determinar la existencia de asociación entre la presencia de anticuerpos y el sexo, las transfusiones sanguíneas, trasplantes previos, hepatitis B, C y anticuerpos anti-HLA. Resultados: la prevalencia de infección con estos virus fue semejante en sujetos sanos y pacientes en espera de trasplante renal. Existió asociación entre IgM anti-CMV, IgG anti-CMV y IgM anti-EBV con el sexo, e IgG anti-CMV con las transfusiones, la seropositividad para la hepatitis C y los anticuerpos anti-HLA clase I. Conclusiones: se hace necesario tomar medidas para evitar el contagio peritrasplante por transmisión sanguínea de los pacientes seronegativos a estos virus pues debido a la inmunosupresión que provocan constituyen un riesgo para el éxito del trasplante renal(AU)


Introduction: Solid organ post-transplant viral infections are the main cause of worldwide morbi-mortality in transplanted patients. In Cuba it has been recently introduced the IgM and IgG anti Epstein Barr (EBV) and anti Citomegalovirus (CMV) antibody detection by ELISA with automatic analyzers as part of the pre transplant studies. Objective: to know population viral infection prevalence and to find possible association between anti EBV and anti CMV antibodies with sensitizing events and anti-HLA antibodies. Methods: An, investigation was carry out using 1179 samples from patients waiting for renal transplant at the Institute of Hematology and Immunology since August 2013 to December 2014. Four enzyme immunoassay (ELISA) heterogeneous type, non-competitive, quantitative and indirect were performed using commercial kits: Cytomegalovirus IgG ELISA, IgM ELISA Cytomegalovirus, Epstein-Barr virus VCA IgG and Epstein-Barr virus VCA IgM. Alloimmunization state anti-HLA class I and II are defined according to studies by ELISA with commercial kits: LIFECODES QuikScreen and LIFECODES B-Screen. Chi square test of independence was used to determine the existence of association between the presence of antibodies and sex, blood transfusions, previous transplantation, hepatitis B, C and anti-HLA antibodies. Results: It was found that the viral infection prevalence was the same as other populations, association of IgM anti CMV, IgG anti CMV and IgM anti EBV with sex and IgG anti CMV with blood transfusions, hepatitis C seropositivity and anti-HLA clase I antibodies. Conclusions : It is necessary to take measures to avoid peritransplant contagion of seronegative patients to theseviruses by blood transmission due to the immunosuppression that they cause, in order to obtain a renal transplant success(AU)

Background: In India, RUBELLA & CMV Antibodies screening is routinely carried out, but due to absence of clear intervention, its value is questionable. This retrospective study evaluated the usefulness of rubella & cmv antibody screening during pregnancy. Methods: Blood is collected from pregnant women & children, tested for rubella and CMV specific IgM Antibody by capture ELISA. The data were analysed to determine the incidence of rubella &CMV infection during pregnancy & in congenital infections. Results: In asymptomatic pregnancy females (n=252) rubella positivity was 1.58% and in woman with BOH (n=110) it was 3.66%, while CMV passivity was 2.9% in both asymptomatic and BOH women. In children (n=100) the overall positivity for rubella and CMV specific IgM antibodies was 7.5% and 12.5% respectively. The incidence of rubella and CMV infections in pregnant women & women with BOH has declined. In children, also the congenital rubella syndrome has declined but for the past 5 years the incidence of CMV infection remained same without any change. Conclusions: The incidence of rubella over the past 5 years can be reduced after giving the rubella vaccine to school going girls & women with reproductive age apart from routine vaccine. But CMV infection is a big problem due to unavailability of vaccine & safe treatment against CMV infection, so routine screening for rubella and CMV should be reserved for women with obstetric complications only.

Abstract: objective: to detect the presence of infection by EBV (Epstein-Barr Virus), CMV (Cytomegalovirus) and HSV-1 (Herpes Simplex Virus type 1) in subgingival samples from HIV- positive patients under HAART (High Activity Antiretroviral Therapy), HIV- positive patients without HAART, HIV-negative patients with chronic periodontitis and healthy controls. Methodology: Crevicular fluid samples of 11 HIV+ patients on therapy were evaluated, 6 without antiretroviral therapy, 7 HIV- negative subjects with chronic periodontitis and 7 periodontally-healthy controls. PI (Plaque index), GI (Gingival Index), PD (probing depth) and CAL (Clinical Attachment Loss) were registered at six sites per each tooth in all teeth and subgingival plaque samples of a tooth were collected per quadrant. Nested PCR was used to detect EBV and endpoint PCR to detect infection by CMV and HSV-1. Results: Clinical parameters showed statistically significant differences between HIV-positive patients and subjects with chronic periodontitis compared with the control group (p<0.05). DNA of EBV was detected mainly in HIV-positive patients under HAART, 91 percent (10/11). DNA of CMV was detected mainly in patients without HAART, 67 percent (4/6), while HSV-1 was observed in 27 percent (3/11) of patients under HAART. In the control group no virus was detected. Coinfection was observed in 50 percent of HIV patients without HAART, 36 percent of HIV patients with HAART and 14 percent of HIV-negative with chronic periodontitis. Conclusion: Viral infection was prevalent in HIV patients under HAART and EBV was the primary viral infection detected in HIV-positive patients with chronic periodontitis.


Resumen: detectar la presencia de infección por VEB (Virus Epstein-Barr), CMV (Citomegalovirus) y VHS-1 (Virus Herpes simple tipo 1) en muestras subgingivales de pacientes VIH-positivos bajo HAART (Terapia Anti Retroviral de Alta Actividad), VIH-positivos sin HAART, pacientes VIH-negativos con periodontitis crónica y controles sanos. Metodología: Se evaluaron muestras de fluido crevicular de 11 pacientes VIH+ bajo terapia, 6 sin terapia antiretroviral, 7 sujetos VIH–negativo con periodontitis crónica y 7 controles periodontalmente sanos. Se registró el IP (Índice de placa), IG (Índice Gingival), PS (Profundidad del Sondaje) y NIC (Nivel de Inserción Clínica) en seis sitios por diente en todos los dientes y se recolectaron muestras de placa subgingival de un diente por cuadrante. Se empleó PCR anidada para detectar VEB y PCR punto final para identificar la infección con CMV y VHS-1. Resultados: Los parámetros clínicos mostraron diferencias estadísticamente significativas entre pacientes VIH-positivos y sujetos con periodontitis crónica comparados con el grupo control (p<0.05). El ADN de EBV fue detectado principalmente en pacientes VIH-positivos bajo HAART con 91 por ciento (10/11). El ADN de CMV se detectó principalmente en pacientes sin HAART, 67 por ciento (4/6), mientras que VHS-1 se observó en 27 por ciento (3/11) de los pacientes bajo HAART. En el grupo control no se detectó ningún virus. La coinfección fue observada en 50 por ciento de los pacientes VIH sin HAART, 36 por ciento de los VIH con HAART y 14 por ciento de los VIH negativos con periodontitis crónica. Conclusión: La infección viral fue predominante en los pacientes VIH bajo HAART y VEB fue la principal infección viral detectada en los pacientes VIH positivos y con periodontitis crónica.

ABSTRACT

O citomegalovírus (CMV) é um vírus altamente prevalente em todo mundo. Em pacientes transplantados, é o agente mais comum de infecção, com uma incidência variando de 20 a 60% e mortalidade podendo chegar a 90%. O aparecimento de infecções no período pós-transplante é determinado pelo perfil sorológico da dupla receptor/doador. O objetivo do trabalho foi determinar a infecção ativa pelo citomegalovírus (CMV) em pacientes transplantados renais utilizando a técnica de PCR em Tempo Real. O estudo foi do tipo transversal, retrospectivo e quantitativo onde participaram 132 pacientes submetidos a transplante renal no Hospital Universitário Walter Cantídio (HUWC) da Universidade Federal do Ceará, que realizaram a pesquisa de CMV pela técnica de PCR em tempo real, no período de janeiro a dezembro de 2012. Foi feita a análise da soroprevalência para CMV do par Doador/Receptor mostrando que a maioria dos receptores (85,6%) e seus respectivos doadores (87,9%) eram soropositivos para CMV antes da realização do transplante. The cytomegalovirus (CMV) is a highly prevalent virus worldwide.On transplanted pacients, he’s the most common agent of infection with 20 to 60% of incidence and mortality reaching 90%. The emergence of infections in the post-transplant period is determined by serological profile of the pair donor/recipient. The study was cross-sectional, retrospective and quantitative attended 132 patients undergoing renal transplantation at University Hospital Walter Cantídio (HUWC) of the Federal University of Ceará, who carried out the research CMV by PCR in real time, from January to December 2012. Background: Rheumatoid arthritis (RA) is characterized by synovial joint inflammation, which often leads to progressive joint destruction and disability. Several other auto-antibodies specific to RA have been found .Among them, antibodies against cyclic citrullinated peptides (CCP) are useful for diagnosing RA. Antibodies to mutated citrullinated vimentin (MCV) were described recently in RA. The aim of this study was to determine the diagnostic values of ACCP compared to anti-MCV and Rheumatoid Factor in rheumatoid arthritis patients. Methods: This study included 92 patients with Rheumatoid arthritis (RA) and 35 matching healthy controls. Blood samples were obtained from patients and controls for Erythrocyte Sedimentation Rate (ESR), C Reactive Protein (CRP), Rheumatoid factor (RF). Anti-CCP2 and anti-MCV were determined using ELISA technique. Results: RA group was significantly higher than control group as regard ESR, CRP, RF, Anti-CCP, and Anti- MCV. Conclusion: It was concluded, compared to ACCP, anti-MCV has approximately the same accuracy for the diagnosis of rheumatoid arthritis but higher than Rheumatoid Factor. Level of Evidence: Level II, prospective study, as per guidelines for authors.

ABSTRACT

Objetivo ­ Identificar a incidência de Citomegalovírus (CMV) em pacientes HIV (Vírus da Imunodeficiência Adquirida) positivos, através de levantamento de dados arquivados de pacientes que apresentaram produção de Imunoglobulina IgG e IgM para CMV e provável positividade para ambos. Métodos ­ Os resultados foram obtidos através de análise de prontuários de pacientes HIV positivos, atendidos em um laboratório de análises clínicas de Jundiaí, São Paulo, cujo atendimento consiste em pacientes da rede particular e do Sistema Único de Saúde (SUS). Os dados foram levantados num retrospecto de 46 meses, entre janeiro de 2011 e outubro de 2014 e foram analisados três parâmetros principais: a média de solicitações de sorologia para HIV, a incidência para CMVM (IgM para Citomegalovírus) e CMVG (IgG para Citomegalovírus) em pacientes HIV positivos e o percentual destes pacientes infectados tanto na rede particular como na rede pública. Resultados ­ No período analisado, 88 pacientes foram diagnosticados com HIV confirmados através de Western Blotting. Destes confirmou-se que 2,27% apresentaram IgM positiva para CMV. Tais pacientes foram atendidos pela rede particular de saúde. Conclusão ­ Isto posto, apesar da TARV (Terapia antirretroviral), ainda há pacientes que se infectam com o CMV. Foi possível avaliar que independente da rede de atendimento o paciente imunocomprometido pelo HIV necessita de atenção especial e acompanhamento minucioso para sobrevida e bem-estar.


Objective ­ To identify the incidence of cytomegalovirus in patients seropositive for HIV, up examinations of patients with IgG and IgM for CMV positivity and possible for both. Methods ­ Through medical records of HIV-positive patients, the data were performed after prior approval of a clinical laboratory in Jundiai, São Paulo, which is to serve the public private network and public network. The data were collected in a retrospective of 46 months between January 2011 and October 2014 and there were three data analysis: the average requests for HIV testing, the incidence for E CMVG CMVM and the percentage of these infected patients in private institutions and the public network. Results ­ Through the medical records, it was found 88 patients with HIV confirmed by Western blotting, it was confirmed that 2.27% were infected with CMV, is have positive IgM and are treated at private. Conclusion ­ That said, despite the HAART (antiretroviral therapy), there are still patients who became infected with CMV, it is possible to assess whether the service network immunocompromised HIV patients need special attention and careful monitoring for survival and well-being.

BACKGROUND: The expression of the SOCS genes in cytomegalovirus (CMV) viremia after hematopoietic stem cell transplantation (HSCT) remains largely unexplored. METHODS: Using quantitative RT-PCR of mononuclear cells, we conducted pairwise comparison of SOCS1 and SOCS3 expression levels among a healthy donor group (N=55), a pre-HSCT group (N=17), and the recipient subgroup (N=107), which were divided according to the occurrence of CMV viremia and acute graft-versus-host disease (aGVHD). RESULTS: Compared to that in the healthy donor group, SOCS1 expression was higher in the CMV+ subgroup, especially in the CMV+GVHD- group, but decreased in the other subgroups. When compared to the expression in the pre-HSCT group, SOCS1 expression was significantly higher in the CMV+ subgroup, especially in the CMV+GVHD+ subgroup. Meanwhile, compared to that in the healthy donor group, SOCS3 expression was significantly lower in all other groups. The CMV-GVHD- subgroup showed significantly lower SOCS3 expression compared to the CMV+ subgroup, the CMV+GVHD+ subgroup, and the CMV+GVHD- subgroup. CONCLUSION: We report differential expression of SOCS genes according to CMV viremia with acute GVHD occurrence after HSCT, suggesting that regulation of SOCS expression is associated with CMV viremia.
